Dhanvantari said: I will explain the cause of abscess; listen, O Suśruta, to that of gulma as well! It arises from consuming food that is stale, excessively hot, dry, rough, or burning.
From lying on uneven beds, improper movements, and various blood vitiations, the corrupted flesh, fat, marrow, and bone become the seat of disease in the abdomen.
That swelling, both external and internal, with severe pain and great suffering, which is round or elongated, is known as the disease called abscess.
Formed by the separate or combined action of the doṣas and blood, it moves in various parts of the body, hard, knotted, and not discharging.
Internally, it is severe and deep, increasing like a tumor; it discharges like an anthill and causes digestive weakness.
It occurs in the navel, bladder, liver, spleen, heart, abdomen, and groin; when present in the heart, there is trembling and intense pain in those regions.
With a dark-red head, irregular maturation, and uneven shape, it is accompanied by loss of consciousness, confusion, distension, oozing, creeping sensations, and noise.
Due to pitta, it appears red, coppery, or blackish, with delirium, fever, and burning; arising suddenly and maturing quickly, it is pale and itchy from kapha.
Distress, chills, stiffness, yawning, loss of appetite, heaviness, delayed onset, lack of proper digestion—these are the mixed symptoms arising from the combined disturbance of all three doshas.
Here, due to the strength of the disorder, there is abnormal stool, with both external and internal signs: black eruptions, dark discoloration, intense burning, pain, and fever.
The signs of pitta are also found in external bleeding, especially in women; and disease can also be caused by blood from injuries by weapons and similar causes.
When arising from a wound and agitated by wind, the blood provokes pitta, producing the symptoms of pitta and blood, and causing abscesses with many complications.
Thus, the variations in complications are recognized according to the site of affliction; for example, if swelling occurs at the navel or bladder, difficulty in urination arises.
Obstruction of breathing and exhalation, spleen disorder, excessive thirst, and throat obstruction occur in the kloma; pain throughout the body and in the heart also arise.
Fainting, darkness before the eyes, cough, palpitations of the heart, and in the sides of the abdomen, as well as the generation of doshas in the abdomen, also occur.
Similarly, pain arises in the thigh-joints, groin, hips, back, and sides; there is pain in the anus and obstruction of wind.
One should recognize in these cases the presence of raw, ripe, or suppurated swelling; some discharge upward from the navel when ripe, others downward from the anus.
Know that abscesses arising from the anus, breast, or navel are due to the moistening of doshas; the combined disturbance of doshas causes changes at their respective sites.
When mature, if located at the navel or bladder, the abscess may rupture either internally or externally; if the abscess is depleted and complications arise, the suppuration occurs within.
There, abscess develops in wicked women and sinners; if the fetus dies in the womb, a hard swelling may also arise.
Pain in the breast, whether present or not, is a sign of external abscess; due to the subtlety of blood in women, it does not occur in maidens.
When wind is enraged or its movement is obstructed, it produces swelling at the root of the penis; reaching the scrotum and groin, it causes excessive swelling of the testicular sac.
By compressing, it causes enlargement of the veins in the testicular sac; the dosha accumulates in the fat there, and the disease manifests in seven forms with swelling.
Due to wind, swelling may occur at the ends of the urinary tract, either externally or internally; it is filled with air, rough to the touch, dry, and causes burning due to wind.
When ripe, due to pitta, it resembles a fig, with burning, heat, and suppuration; due to kapha, it is severe, heavy, unctuous, itchy, hard, and causes little pain.
The swelling is black, covered with eruptions, and shows signs of growth due to blood; with kapha and fat, the swelling increases, soft and resembling a ripe palmyra fruit.
For one who habitually retains urine, as the urine moves there, the absence of greed and full steadiness are disturbed, and the gentle flow becomes agitated.
Difficulty in urination arises below, with swelling of the scrotum and testicles, accompanied by aggravation of wind, and by immersions in cold water.
Also, by retaining feces and urine, and by irregular movements of the limbs, when the vital energies are disturbed and depleted within the body—
The wind, having become disordered, drives the blood downward; then, at the joint, it produces swelling resembling a knot.
