Diseases of wind, urinary stones, leprosy, diabetes, abdominal swelling, foul odor, hemorrhoids, and grahaṇī—these eight great diseases are extremely difficult to overcome.
Dhanvantari said: Now, O Suśruta, listen to the causes of urinary retention. The bladder, the neck of the bladder, the penis, the loins, the testicles, and the anus—
Are said to be supported by single channels, situated in the cavities of the rectum and bones; even the bladder faces downward with the openings of the urinary veins.
From the sides, fine streams constantly fill it; entering through these, they produce twenty kinds of disorders.
Urinary retention and diabetes, both severe, affect the vital spots, causing pain in the bladder, groin, and penis, which is mild but frequent.
In the wind type, urine is scanty; in the bile type, it is yellow and always burning; in the phlegm type, it is bloody, with heaviness and swelling of the bladder and penis.
The urine is slimy, obstructed, and affected by all impurities; when wind blocks the opening of the bladder and dries it out—
Then the urine, mixed with bile, phlegm, or semen, gradually forms a dreadful stone, yellow like cow's bile.
All these are based in phlegm; the early sign of this is swelling of the bladder and intense pain around the affected area.
There is retention of urine in the bladder, painful urination, fever, and loss of appetite; the common symptoms are pain in the navel, perineum, bladder, and head.
The urine is copious when the passage is open; when blocked, it is passed with difficulty, clear like the gemstone gomedaka.
Due to agitation, there is pain with blood and flesh in the tract; there, afflicted by wind, the patient bites his teeth and trembles.
He grasps his penis and navel, suffering greatly; with wind, he passes stool, and urinates frequently, drop by drop.
His stone is dark and rough, sometimes sharp like thorns; with bile, the bladder burns as if being cooked by heat.
The stone is like the seed of bhallātaka, red, yellow, or white; the bladder, pressed by phlegm, is cold and heavy.
A large, smooth stone, honey-colored or white, occurs in children, and especially among them in greater numbers.
Because of the smallness of the channel and the ease of grasping and expelling, a person is comfortable; but when semen is retained, a large stone (calculus) of semen is formed.
If the semen is displaced from its place or not expelled, wind enters between the two testicles, dries up the semen, and that dried semen becomes a stone (calculus).
This causes pain in the bladder, difficulty in urination, and swelling; as soon as it arises, it dries up and dissolves.
When afflicted, there is fever and cough, and in this case, the stone is like gravel; or, broken by wind, it moves in the reverse direction.
It is expelled together with urine, or, moving against the flow, it is digested; when wind is agitated at the mouth of the bladder, it causes retention of urine.
There is retention of urine, pain, itching, and sometimes swelling; covering the bladder, it rises up, large and displaced, to the region near the anus.
There, it causes pain, burning, throbbing, and twisting; when the bladder is pressed, urine is passed only drop by drop.
This obstruction of the stream is known as 'wind in the bladder'; it is difficult to overcome, and the second, stronger wind is even more difficult.
When wind occupies the space between the rectum and the bladder, it produces a hard, knotty, and elevated mass like a pestle.
This is called 'wind-pestle'; it is produced by the wind of Vishnu's channel; being defective, coiled, and intense, the wind in the bladder causes severe pain.
Binding up the urine, it moves about, causing stiffness, twisting, and heaviness; the urine is passed little by little, or only occasionally.
This is called 'wind-coil'; when the urine is retained for a short time, it does not come out, or is obstructed, and after urination, there is little pain.
When, due to retention, the wind is obstructed and whirls about, then the lower abdomen below the navel fills with urine.
It causes intense pain, distension, inability, and accumulation of waste; this urine is due to a defect in the opening of the stomach or by wind.
