Hear now the sacred teachings as passed down from Dhanvantari, the divine healer, to the wise Suśruta, a treasury of remedies for many afflictions of the body. To begin, one seeking to restore health should, for ten days, partake daily of ten paippālika fruits with milk, gradually increasing the number each day. After reaching the peak, the dose is then reduced in the same manner. For those who consume rice cooked in milk for a thousand days, nourishment and longevity are assured, and diseases of the spleen and abdomen are banished. Swelling is alleviated by clarified butter prepared with the decoction and paste of punarnavā; this may be used with cow’s urine, or with pippalī and milk, or guḍa and harītakī in equal measure, or with viśva—each of these is beneficial for those suffering from swelling. Castor oil, when prepared with balā and milk and then consumed, removes abdominal distension, pain, and the accumulation of fluids. For disorders of the abdomen, a paste of pathyā fried in mustard oil and mixed with black salt is highly effective. If glandular swellings arise, the root of nirguṇḍī, when applied, destroys them. Fomentation with the leaves of smuhī and gaṇḍīrikā also removes tumors. Goiter is treated by applying a paste of hastikarṇapalāśa to the neck, along with dhattūra, eraṇḍa, nirguṇḍī, varṣābhū, śigru, and mustard. Chronic and severe elephantiasis is destroyed by a poultice, while a mixture of śobhāñjana, rock salt, and hiṅgu cures abscesses. Wounds and ulcers of all kinds are healed by a paste of śarapuṅkhā with honey; similarly, an application of nimbapatra treats swelling and ulcers. Triphalā, khadira, dāru, and nyagrodha cleanse wounds thoroughly. Whenever a fresh wound is encountered, the physician should irrigate it immediately, even if it causes pain. When wounds are caused by external agents, the physician should apply a slightly warm mixture of ghee and honey, combined with licorice. Cooling treatments that destroy excess heat, blood, and bile are recommended, such as a decoction prepared from bamboo, castor, and dog-tooth grass. When this is taken with asafoetida and rock salt, it expels blood retained in the bowels; or it may be taken with the juice of barley, jujube, and horse gram, without oil. Foods made from barley, or barley mixed with rock salt, may be consumed. The juices of karanja, arishta, and nirguṇḍī destroy worms in wounds. Guggulu, prepared with triphala powder and formed into pills, can be taken without restriction to remove obstructions and is effective for wounds and swelling. The juice of dūrvā grass or kampillaka, together with the bark of dārvā, forms a principal remedy for healing wounds. Then Dhanvantari addressed Suśruta further, saying, "Hear the treatment for fistula and related diseases. After opening the fistula with a surgical instrument, treat it as a wound. A mixture of guggulu, triphala, and vyosha in equal parts, combined with ghee, cures painful, diseased fistula and also bhagandara. Oil prepared from the juice of nirguṇḍī removes diseased fistula and benefits those afflicted with skin diseases, whether used for drinking, anointing, or as a nasal application. Pills made from guggulu, triphala, kṛṣṇā, and tripañca in equal parts are beneficial for tumors, swelling, and bhagandara. In cases of upadaṃśa, venesection in the middle of the penis brings purification. The ulcer must be carefully protected, for it can cause the destruction of the glans. A decoction of paṭola, nimba, triphala, and guḍūcī, together with guggulu and khadira, destroys upadaṃśa. Triphalā, mixed with honey and roasted in a pot, when applied as a paste, immediately heals the wound of upadaṃśa. A paste and decoction of triphalā, nimba, bhūnimba, karanja, khadira, and other herbs, cooked with ghee, is an excellent remedy for this disease. When a fracture is recognized, it should first be irrigated with cold water, then a cooked ointment should be applied, and the limb bound with kusa grass. The patient should be nourished with black gram, meat, ghee, milk, soup, and water with barley. Garlic, honey, ghee, and sugar paste should be consumed; for those with broken, dislocated, or fallen bones, this ensures rapid union. A mixture of aśvattha, triphalā, vyosha, and varabhi, prepared together and combined with guggulu, restores fractured joints. For all types of leprosy, emesis, purgation, and bloodletting are prescribed, along with the use of vacā, vāsā, paṭolā, nimbā, kalinī, and twigs of bark. A decoction possessing the qualities of alleviating wind and heart disorders, drunk with honey, should be used for purgation with trivṛt, karṇaphala, and trikatu. Oil prepared with manaḥśilā and black pepper destroys leprosy; this ointment is effective for all types of leprosy, as is the mixture of śivā, pañcaguḍa, and rice. Leprosy is also alleviated by applying a paste of karañja and iḍagaja with cow’s urine; massaging with karavīra and anointing with oil also removes the disease. A paste made from turmeric, malaya, rāsnā, guḍūcī, iḍagaja, āragvadha, and karañja is an excellent remedy for leprosy. Manaḥśilā, viḍaṅga, vāgajī, and mustard seeds, ground with karañja and urine, form a paste that removes leprosy as the sun removes darkness. Viḍaṅga, iḍavacā, kuṣṭha, niśā, sindhūttha, and mustard seeds, ground with sour urine, make a paste that destroys ringworm and leprosy. Prapunnāṭa seeds, subīja, dhātrī, sarja resin, snuhī, and sauvīra, when ground together, form an excellent powder for ringworm. The leaves of āragvadha, ground with buttermilk, destroy ringworm, scabies, leprosy, and sidhmā. Warm vāgajī, when drunk, along with a diet of milk, sesame oil, triphalā, honey, vyosha, bhallātaka, and sugar—these seven together strengthen the body, promote intelligence, remove leprosy, and increase desire. Powder of viḍaṅga, triphalā, and kṛṣṇā, taken with honey, destroys leprosy, worms, urinary tract diseases, ulcers, and fistula-in-ano. Whoever consumes abhayāriṣṭa, ariṣṭa, āmalaka, and niśā will certainly conquer all types of leprosy and skin diseases—of this, there is no doubt. Thus, the divine wisdom of healing is revealed, guiding all who seek health and wholeness.
